Medical providers can report a disease by phone or fax.

CMR Form and Diseases that Must be Reported

Clinicians are legally required to report specific diseases to the San Francisco Department of Public Health. The list of reportable diseases can be found on page 2 of the Confidential Morbidity Report form.

There’s now a separate form for reporting COVID-19 cases.
