Flu Vaccine Programs: Ordering Information for Health Care Providers

Providers should contact their regular distributors or the vaccine manufacturers. Providers who meet eligibility criteria (local public health centers, nonprofit organizations, and community organizations who serve clients who are low-income, under-insured or uninsured) may qualify for free vaccine from our Influenza Vaccine Distribution program (see below).

Seasonal Flu Vaccine Distribution Program for Clinics Serving Low Income or Uninsured Clients

Open enrollment for the Flu Vaccine Distribution Program is limited to once per year.  All 2022-2023 program applications are due by Friday, August 19, 2022.  If your organization is interested in applying to receive vaccine from SFDPH, please review the program guidelines.  If your organization meets the eligibility criteria, contact the SFDPH immunization Program at immunization@sfdph.org or (415) 554-2955 to request the application materials.

Flu Program Participant Requirements and Resources

Program participants agree to follow California Department of Public Health (CDPH) standards for storage and handling of vaccine.  Prior to submitting your application, your Primary Vaccine Coordinator and two Back-Up Coordinators must thoroughly read the current SFDPH Vaccine Management Resource Guide.  This guide is a compilation of resources designed to help your clinic with vaccine administration, ordering, storage and handling.

Two usage reports are due each flu season:

  • Mid-Year Usage Report due in January/Feb
  • Final Usage Report due in July

Vaccine Storage and Handling

SFDPH Vaccine Management Resource Guide – Must be reviewed annually by the Vaccine Coordinator and Backup Coordinator

Key Staff Contacts Form – Post a copy of this on your storage unit and update it with any staffing change

Emergency Vaccine Management Plan – Post a copy of this on your storage unit and update annually or with any staffing change

EZIZ Trainings – Both Storage and Handling trainings must be completed annually by Vaccine Coordinator, Backup, and any other staff monitoring temperatures

SFDPH Vaccine Storage and Handling Incident Summary Form – Complete this form if your vaccine storage unit temperatures go out of range

Vaccine Requests and Transport

Vaccine Request Form – To be submitted by current program members when vaccine is needed throughout the year

Vaccine Return Form – Complete this form and bring it when returning vaccine to our program

SFDPH Vaccine Transport Guide – Please follow this guidance when picking up or transporting vaccine

Vaccine Accountability

2022-23 Influenza Vaccination Authorization Record – Optional resource for partners to track flu doses administered and obtain patient authorization
